- Recent passport-size photograph

- Address proof of premises
Premises Address Proof
- Latest electricity bill (≤ 2 months old)
- Registered rent agreement (if rented)
- NOC from owner of premises
- Property tax receipt
- Municipal khata copy
- Consent letter (if shared premises)
Restaurant / Cloud Kitchen
- Form B duly filled and signed
- Photo ID & address proof of authorised signatory
- Proof of premises + NOC of owner
- Kitchen layout with dimensions
- List of food categories served
- Water-test report (if water is used in food)
Manufacturer / Processor
- Blueprint of unit signed by authorised signatory
- List of machinery with installed capacity (kg/L per day)
- Food Safety Management System (FSMS) plan
- NOC from municipality or local body
- Source-of-raw-material declaration
- Recall plan (for Central Licence)
Importer / Exporter
- Import Export Code (IEC)
- List of food products to be imported / exported
- Source-country & product-category declaration
- Authorisation letter from manufacturer (for import)
- Photo ID & address proof of authorised signatory
- Proof of premises (head office)
Company / LLP / Partnership
- Certificate of Incorporation / LLP agreement / Partnership deed
- PAN of the entity
- PAN & Aadhaar of directors / partners
- Board resolution authorising signatory
